Advanced Nurse Practitioner (ANP)/Candidate Advanced Nurse Practitioner (cANP) - Diabetes: Creating a partnership approach with streamlined communication pathways and care transition protocols between Mercy University Hospital and community services. Responsibilities include developing standardized care pathways, establishing an efficient case management system, and demonstrating accountability and responsibility for professional practice. Eligible applicants must be registered in the Advanced Nurse Practitioner division of the Nursing and Midwifery Board of Ireland Register or be eligible to register; have a broad base of clinical experience; and demonstrate the competences relevant to the specialist area of advanced practice. cANP education criteria include eligibility to undertake a Master's Degree or currently undertaking a Master's Degree in Nursing (Advanced Practice Pathway).
