Advanced Nurse Practitioner (ANP) / Candidate Advanced Nurse Practitioner (cANP) - Diabetes Permanent, Full time Role The ANP/cANP Diabetes will be: Professionally accountable to the Director of Nursing. Clinically accountable to the supervising Consultant. ANP/cANP Responsibilities: Creating a partnership approach with streamlined communication pathways and care transition protocols between Mercy University Hospital (MUH) and community services, ensuring an unbroken care journey for patients. Development of standardized care pathways and protocols that provide for timely referral to services appropriate to each individuals needs, with the goal of aiming for high-quality, person-centred care delivery. Establish an efficient case management system that optimises patient access to services, reduces Emergency Department visits, and minimizes readmissions. This system will coordinate services and resources, facilitating effective care planning and transitions. Demonstrating accountability and responsibility for professional practice as a lead healthcare professional. Articulating safe boundaries and engaging in timely referral and collaboration for those areas outside the scope of practice, experience, and competence. Conducting a comprehensive holistic health assessment using evidenced based frameworks to determine diagnoses and inform autonomous advanced nursing care. Providing leadership in the translation of new knowledge to clinical practice. Educating others using an advanced expert knowledge base derived from clinical experience, on-going reflection, clinical supervision and engagement in continuous professional development. Communicating effectively with the healthcare team through sharing of information in accordance with legal, professional and regulatory requirements. Demonstrating leadership in professional practice by using professional language (verbally and in writing) that represents the plan of care, which is shared with the person and other members of the inter-professional team. Promoting a culture of quality care. Proactively seeking feedback from persons receiving care, families and staff on their experiences and suggestions for improvement. Implementing practice changes using negotiation and consensus building, in collaboration with the multidisciplinary team and persons receiving care. Demonstrating clinical leadership in the design and evaluation of services. Engaging in health policy development, implementation, and evaluation. Identifying gaps in the provision of care and services pertaining to the ANP/cANPs area of advanced practice and apply the best available evidence. Eligibility Criteria ANP Criteria - Eligible applicants will be those who on the closing date for the competition: Are registered in the Advanced Nurse Practitioner division of the Nursing and Midwifery Board of Ireland Register maintained by the Nursing & Midwifery Board of Ireland (NMBI), OR be eligible to register in the Advanced Nurse Practitioner division of the Nursing and Midwifery Board of Ireland Register maintained by the NMBI by meeting the criteria for registration as an Advanced Nurse Practitioner as specified Have a broad base of clinical experience relevant to the advanced field of practice. Demonstrate the competences relevant to the specialist area of advanced practice. Possess the requisite knowledge and ability including a high standard of suitability and clinical, professional and administrative capacity to properly discharge the functions of the role. cANP Criteria - Eligible applicants will be those who on the closing date for the competition: Be a registered nurse with the NMBI or entitled to be so registered. Be registered in the division(s) of the NMBI Register for which the application is being made or entitled to be so registered, OR in recognition of services that span several patient/client groups and/or division(s) of the register, provide evidence of validated competences relevant to the context of practice. Have a broad base of clinical experience relevant to the advanced field of practice. Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, professional and administrative capacity to properly discharge the functions of the role. cANP Education Criteria: Be eligible to undertake a Masters Degree (or higher) in Nursing or a Masters Degree, which is relevant, or applicable, to the advanced field of practice. The Masters programme must be at Level 9 on the National Framework of Qualifications (NFQ) (Quality & Qualifications Ireland - QQI), or equivalent. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice, in addition to clinical practicum, OR Be currently undertaking a Masters Degree in Nursing (Advanced Practice Pathway) or be eligible to register to undertake additional Level 9 NFQ (QQI) specific modules of a Masters Degree in Nursing (Advanced Practice Pathway) within an agreed timeframe. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice, in addition to clinical practicum, OR Possess a Masters Degree (or higher) in Nursing or a Masters Degree which is relevant, or applicable, to the advanced field of practice. The Masters programme must be at Level 9 on the NFQ (QQI), or equivalent. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice, in addition to clinical practicum.
