Who we`re looking for Client Care Coordinator with following Education/Experience requirements: QQI in Healthcare or related One year experience in home care, health care or senior-related industry required, or equivalent Must possess a valid drivers license and have access to a car Demonstrate discretion, integrity and fair-mindedness Demonstrate excellent written and verbal communication skills a Ability to establish collaborative working relationships with the team Ability to work independently and meet deadlines. Ability to plan, organize and prioritize Ability to travel on certain occasions as the business requires. What you`ll do The Client Care Coordinator is expected to perform a variety of duties including Care consultations with potential clients and family members, client/CAREGiver introductions quality assurance visits with existing clients. use a consultative sales approach to determine individual clients needs provide solutions and create a tailored service plan for clients Reflect the Home instead core values. Demonstrate open and effective communication with colleagues Focus on professional and personal development. Work with other team members to coordinate various aspects of a clients care Conduct client/CAREGiver introductions with every new client and with every new CAREGiver. Create and maintain client and responsible party records documenting all quality assurance meetings. Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers. Skills: Healthcare QQI Level5
