Join the Cosan Nua East team as a Case Management Worker to provide support to individuals and families from diverse backgrounds, residing in IPAS accommodation in East Ireland.
This role offers a holistic approach to case management, prioritizing harm minimization, care, and dignity for service users. You will work across a 5-day week, with flexible annual leave and benefits.
Key Responsibilities:
- Work directly with service users to achieve best outcomes using the Depaul Support Toolkit.
- Develop and maintain relationships with local authorities and other agencies for interagency approaches.
- Assess clients' housing needs and support their progression into private rented accommodations.
- Advocate and empower clients to overcome barriers and progress into independent living.
- Record and store client information according to data protection regulations and confidentiality policies.
- Support managers in reviewing service delivery and meeting KPIs.
- Collaborate with teams to ensure service user needs are met and core objectives are achieved.
Requirements:
- Social Studies/Case Management Accredited Qualification
- Full Valid Driver's Licence and access to a car
- At least 2 years of experience as a case manager with complex needs service users
- Proficiency in Excel, Word, and Cloud applications (Salesforce)
- Experience in liaison with agencies, outreach, and tenancy sustainment
- Good knowledge of immigration systems, legislation, and nationality matters in Ireland
- Excellent communication, organizational, and advocacy skills
Desirable: Knowledge of another language, experience in working with families, and understanding of health and safety issues.
