Job Title: Case Manager / Social Care Leader Location: Bray, Wicklow Services: Disabilities Salary: Competitive, in line with HSE pay scales Working Hours: Full Time Monday Friday Contract: Fixed-Term Contract About: Our client is a dedicated Disability Service Team committed to providing high-quality, person-centered care. They collaborate with stakeholders to enhance service delivery and meet the evolving needs of individuals with disabilities. Roles and Responsibilities: Work as part of the Disability Service Team to develop and manage service structures. Build relationships with relevant stakeholders to maximize service provision. Inform service development and prioritize needs where gaps are identified. Develop individualized service options in consultation with clients, families, and relevant professionals. Present costed business cases for service provision as needed. Coordinate and attend multidisciplinary case management reviews. Foster links with statutory, voluntary, and private service providers. Deputize for the Disability Manager when required. Serve as Assessment/Liaison Officer (LO) under the Disability Act 2005 when needed. Maintain up-to-date written and electronic records in accordance with professional standards. Efficiently manage budgets, resources, and prepare financial reports as necessary. Participate in research, development, and service evaluations. Qualifications and Experience: Significant experience in Disability Services and case management. Proven experience working as part of a multidisciplinary team in Disability Services. Experience in service coordination, placement, and discharge liaison. Skills, Competencies, and Knowledge: In-depth knowledge of the Disability Act 2005, ADM (2015), Health Act 2004 & 2007, and HIQA regulations. Understanding of health service structures, social care services, and disability sector services. Proven ability to implement change, evaluate information, and solve complex problems. Strong decision-making skills with the ability to explain decisions clearly. Excellent communication, interpersonal, and report-writing skills. Strong planning, organizational, and leadership abilities. Commitment to delivering high-quality service and promoting continuous improvement. Why Join?
