This a hugely varied geographical area providing community-based services from Baggot Street in the city centre of Dublin to Carnew in South Wicklow. These services are delivered through the HSE, and its funded agencies, to people in local communities, as close as possible to people’s homes. Location of the Post: A panel may be formed for Clinical Nurse Specialist (Neuro-Rehabilitation) as a result of this campaign from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled. Purpose of the Post: The purpose of this Clinical Nurse Specialist (Neuro-Rehabilitation) post is to deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. The successful candidate will work as a key member of the multidisciplinary Community Neuro-Rehabilitation Team providing a person-centred quality service to service users/clients. This specialist practice will encompass a major clinical focus which comprises assessment, planning, delivery, and evaluation of care given to the service user/clients and their families. The Clinical Nurse Specialist (Neuro-Rehabilitation) together with the interdisciplinary team will provide a specialist neuro-rehabilitation service for persons with acquired and progressive neurological conditions. The CNS will act as a liaison between Community Neuro-Rehabilitation Services, Primary Care, Community services, and other agencies. The appointment of the CNS is an essential element in the provision of effective, high-quality, integrated care for those with neuro-rehabilitative needs. The post holder will be responsible for planning, developing, coordinating, and evaluating the service to meet the needs of individuals and the objectives of the CNRT. The post involves working autonomously and as part of an interdisciplinary team within a community setting. Caseload The Clinical Nurse Specialist (Neuro-Rehabilitation) will focus on service users under the care of the Community Neuro-Rehabilitation team. The post holder’s practice is based on the five core concepts of Clinical Nurse Specialist (Neuro-Rehabilitation) role as defined by the NCNM 4th edition (2008) in order to fulfil the role. The concepts are: Clinical Focus (Direct and Indirect Care) Education and Training Audit and Research Consultancy (including leadership in clinical practice) Recruitment Campaign This is an exciting opportunity to be part of an inter-disciplinary Community Neuro-Rehabilitation Team in CHO 6. This is a fulfilling role with team members being part of a multi-professional team providing specialised, intensive rehabilitation to those with neuro-rehabilitative needs as close to home as possible. The CNRT will provide specialist, community-based Neuro-Rehabilitation. Key to the success of the CNRT will be an individual health professional’s ability to provide discipline-specific expertise and care, whilst working in an autonomous manner. Background Context to Neuro-Rehabilitation Strategy As part of the 'National Strategy and Policy for the Provision of Neuro-Rehabilitation Services in Ireland – Implementation Framework 2019-2021, the HSE are developing networks of Neuro-Rehabilitation Services. The overarching aim of the Strategy is the development of Neuro-Rehabilitation services to improve outcomes for people by providing safe, high-quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. This MCRN will bring together tiered levels of specialist Neuro-Rehabilitation services promoting transparent equitable access to neuro-rehabilitation services. Each MCRN has a Network Rehabilitation Coordinator that acts as the single point of entry for all referrals into the network. How will the CNRTs personal and professional development be supported?
