Seeking a Clinical Nurse Specialist (Neuro-Rehabilitation) to deliver person-centred care to individuals with acquired and progressive neurological conditions in a community setting.
HSE Community Healthcare East is one of nine Community Healthcare organisations providing services to Dublin South, Dublin South East and Wicklow.
The organisation delivers community-based services through the HSE and its funded agencies, aiming to provide care as close to people's homes as possible.
The Clinical Nurse Specialist (Neuro-Rehabilitation) will work as a key member of the multidisciplinary Community Neuro-Rehabilitation Team, providing a specialist neuro-rehabilitation service.
- Deliver care in line with the five core concepts of the Clinical Nurse Specialist (Neuro-Rehabilitation) role.
- Work autonomously and as part of an interdisciplinary team within a community setting.
- Focus on service users under the care of the Community Neuro-Rehabilitation team.
The post holder will be responsible for planning, developing, coordinating, and evaluating the service to meet the needs of individuals and the objectives of the CNRT.
- Provide discipline-specific expertise and care.
- Work in an autonomous manner.
The HSE is developing networks of Neuro-Rehabilitation Services as part of the National Strategy and Policy for the Provision of Neuro-Rehabilitation Services in Ireland.
The overarching aim of the Strategy is to improve outcomes for people by providing safe, high-quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity.
