Dublin South Kildare and West Wicklow Community Healthcare Organisation is one of nine HSE Community Healthcare organisations.Skills and AttributesCommitment to providing a quality servicePlanning and managing resourcesDescriptionPlease Note:CVs will not be accepted.All correspondence will be via Rezoomo only.Details of Service:As part of the 'National Strategy and Policy for the Provision of Neuro-Rehabilitation Services in Ireland – Implementation Framework 2019-2021', the HSE is developing networks of Neuro-Rehabilitation Services. The overarching aim of the Strategy is to improve outcomes for people by providing safe, high-quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. These services are configured into population-based Managed Clinical Rehabilitation Networks (MCRNs). The new model introduces a multi-tiered system, with access to services based on clinically assessed need, allowing for referrals to a network of services.The Community Neuro Rehabilitation Team (CNRT) is an interdisciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This multi-professional team includes a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing, and Administrative staff. The team provides assessment and intensive therapy to people in their homes and in a clinic-based setting for up to 12 weeks, improving quality of life and supporting neuro-rehabilitative needs.As part of the Managed Clinical Rehabilitation Network, the CNRT will play a significant role in evaluating the service and contributing to the development of the team and the wider network.Grade: Speech & Language Therapy ManagerPlease refer to the attached documents for further information. #J-18808-Ljbffr
