Job Description

The post holder will be the clinical lead for governance in Folds Housing with Care Schemes. This role is of pivotal importance to ensure clear lines of clinical accountability across our Care Services.Main ResponsibilitiesExercise high levels of judgement, discretion and guidance in the delivery of housing with cares service within a clinical governance framework.Develop and implement quality improvement initiatives to enhance residents’ safety and overall outcomes.Provide specialist expertise and advice, demonstrating a sound understanding of the issues relating to care of older people.Ensure safe practice in medication management, promoting effective care and preventing, detecting, communicating and learning from adverse incidents and near misses.Develop and deliver customised training programmes to colleagues in accordance with the clinical needs of Fold Housing.Monitor and improve standards through resident’s feedback, scheme audit, evidence-based practices, teaching and supporting the senior care team and the provision of skills and knowledge development.Contribute to the management and investigation of cases regarding poor clinical practices.Complete annual medication supervisions as requested by the Director of Care Services.Perform clinical audits in accordance with HIQA requirements.Complete the annual review required to be submitted to HIQA on an annual basis.Provide nursing leadership within the housing schemes.Complete clinical assessments for new residents and residents returning from hospital to Housing with Care.Secure and sustain high-quality, cost-effective services which deliver positive outcomes.Contribute to effective multi-disciplinary and interagency approaches to the delivery of high-quality services.Collaborate with multidisciplinary teams to promote best practices and ensure consistent quality of care.Clinical GovernanceProvide nursing service/clinical input to the development and implementation of strategies and systems for the continual improvement of services including clinical governance, risk management, health and safety and resident care and the resident experience.Conduct regular audits and assessments to identify areas for improvement and implement corrective actions.Assist in the investigation of medical related accidents complaints and grievances within the service.Participate in HIQA inspections and audits, prepare necessary documentation and reports.CommunicationCommunicate effectively with residents, colleagues and other related personnel face to face, by telephone and using virtual systems when appropriate.Use and maximise the organisation’s IT and communication systems.Compile legible, coherent and well-constructed written reports.Team DevelopmentProvide customised training courses to the Senior Care Workers team and Care Assistant on HIQA requirements and best practices in clinical care.Develop course content to the required level of certification.Encourage participant engagement and motivation to apply learning outcomes.Review and evaluate training content to ensure quality training and continuous improvement.Maintain and promote positive colleague’s relationships within Care Services and with key stakeholders.Management of Professional/GovernanceStay informed of changes in HIQA regulations and industry trends to ensure ongoing compliance.Deputise and act as Person In Charge in the absence of Housing with Care Manager.Review and update policies in line with changes to statutory requirements or as instructed by the Director of Care Services and Housing.Develop and implement systems to monitor the quality of medication practice through audit and measurement of clinical risk, complaints, benchmarking and ensure changes to practice are introduced and monitored where appropriate.Act as a change agent, developing clinically effective practice through the effective utilisation and integration of evidence.GeneralColleagues are required to be flexible in their work and attitude and to co-operate with their colleagues to ensure the efficient, effective and economic operation of the service.FOLD Housing has a responsibility to ensure that employees are not treated less favourably on the grounds of their sex; marital status; religious belief; political opinion; race; disability; sexual orientation; age or any other statutory legislation.This job description is not definitive or restrictive and may be amended to facilitate changes in the better organisation of FOLD Housing’s activities and business objectives.FOLD operates a ‘No Smoking at Work Policy’.PERSON SPECIFICATIONEDUCATION & EXPERIENCEThird level nursing qualification – and registration with an Board Altranais. Minimum of 3 years post registration experience at a senior clinical management level in Care of the Elderly.SPECIALIST KNOWLEDGEExpert knowledge and understanding of HIQA standards.Able to set and monitor standards.Ability to demonstrate experience of corporate and clinical governance.Experience of audit initiatives and other quality initiatives.Experience of research activity including initiating, developing or implementing research in practice.Knowledge of National/local developments in area of specialist interest.Experience in diversional therapy techniques for older persons.Hands-on care skills.Human resources management and/or supervisory management skills.SKILLS AND ABILITIESGood oral and written communication skills.Good organisational and administration skills with strong attention to detail.Presentation skills.High professional standards.Ability to work collaboratively in a team environment.Proficiency in relevant software applications for documentation and reports.PERSONAL QUALITIESConfident communicator.Empathy.Integrity.Commitment.Reliability.Self-motivated.Able to work as part of a team.Ability to work flexible hours as needed.Full driver’s licence and access to the use of a car.Location: Block A, The Crescent Building, Northwood Office Campus, Santry, Dublin 9 #J-18808-Ljbffr
