Dublin Cluain Mhuire is a national center of innovation in public mental health service provision delivering a community based adult mental health service for the South East Coast of Co. Dublin to a catchment area of some 182,000 people approximately.In addition to general adult mental health services, Cluain Mhuire provides early intervention in psychosis, ADHD, gender and general hospital mental health services. Clinical Nurse Manager 3 (Team Coordinator for the Crisis Resolution Service incorporating the acute Day Hospital) Full Time (37.5hrs) Permanent Post Saint John of God Community Services, Community Mental Health Services Cluain Mhuire Service are now inviting applications for the post of Team Co-ordinator (Clinical Nurse Manager 3) for the Crisis Resolution Service incorporating the acute Day Hospital. Role Summary The CNM3 post in the Crisis assessment Service will act as a Team Co-ordinator for the Crisis resolution service.They will co-ordinate service development and provision across the Crisis assessment team and the Acute Day Hospital.They will assist the service in meeting the requirements of the national clinical programme for crisis services and they provide professional/clinical leadership in their designated areas of responsibility. To oversee the management of resources including staffing and staff development. To facilitate communication across the healthcare teams. The post of Team Co-ordinator will be a pivotal role in service planning and development, co-ordinating and managing activity and resources within the area(s) of responsibility. The Team Co-ordinator responsibilities include: overseeing the quality of care, ensuring compliance with regulatory standards and adherence to legislative requirements, the development and implementation of policy and procedures, the monitoring of activity and the delivery of agreed levels of service for the designated area(s) and the promotion and audit of evidenced based standards of care. The CNM3/team co-ordinator will play a key role in providing clinical and professional leadership and promoting the values of the organisation.The successful candidate, dependent of service need, may be required to complete clinical duties in the course of their role. The Team Co-ordinator will be changeable and in line with service need at any given time. Minimum Requirements ELIGIBILITY CRITERIA Candidates must, at the latest date for receipt of completed application forms for the post possess: 1.Qualifications and/ or experience Are registered in the Psychiatric division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnimhseachais na hireann) or entitled to be so registered. And Have at least 5 years post registration experience of which 2 must be as a Clinical Nurse Manager or in a related area. And Candidates must demonstrate evidence of continuous professional development at the appropriate level. And Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role. 2.Health A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service. 3.Character Each candidate for and any person holding the office must be of good character. 4.Age Age restrictions shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation Act, 2004).A candidate who is not classified as a new entrant must be under 65 years of age on the first day of the month in which the latest date for receiving completed application forms for the office occurs. Principle Duties and Responsibilities Coordinating Clinical Team 1.Ensure routine referrals are processed efficiently within the Crisis Resolution Service operational policy. 2.Liaise with Primary Care Teams (GPs and primary care professionals), and other relevant local community services, external agencies, self-help and other community resources. 3.Co-ordinate agreed CRS clinical inputs and ensure all clinical / waiting list/admission and discharge documentation are completed by clinicians. 4.Monitor monthly figures for the allocated MDT and ensure a plan is in place for delayed discharges/transfers. 5.Assess, monitor and escalate clinical risks relating to waiting lists/discharge lists/Clinical issues and liaise with the consultant, Assistant Director of Nursing or nominated other, regarding any risk issues and communicate risk management plans to clinical team 6.Ensure all clinical incidents are reported and escalated appropriately and in a timely manner 7.Monitor team members workloads in line with agreed discipline-specific workload guidelines (as agreed with relevant heads of discipline) in order to facilitate equitable workload distribution and appropriate service throughput for the CRS. 8.Co-ordinate team members leave, in accordance with the protocols agreed with relevant line managers so as to ensure that there is an adequate number of staff on duty to deliver the required level of service. 9.The Team Coordinator will be required to work with members of the Multi-Disciplinary Team to implement the strategic objectives for the St John of God Community Mental Health Services. 10.Maintain good staff relations within the team and promote good communication with all relevant Staff and Departments 11.Give support and counsel to the multi-disciplinary team as necessary and take action in accordance with agreed policy, if necessary 12.In collaboration with the clinical team and QPS Advisor, lead the development of local clinical operational procedures in line with the HSE CRS Model of Care 13.It would be expected in the normal course of events that the role will evolve as professional and service demands change.In particular, the profile will need to be adapted at local level to meet specific requirements in relation to the management of specific functional responsibilities. Supervision/Management 1.Provide clinical supervision to student nurses / nursing as directed by the Assistant Director / Director of Nursing 2.Line manage nursing staff as directed by the Assistant Director / Director of Nursing 3.Contribute to the ongoing development of the nursing department 4.Escalate all issues related to Nursing to the Assistant Director/Director of Nursing. Clinical Practice 1.To assess the needs and collaboratively prepare care plans for people attending the service.To deliver care in line with best nursing practice/ research and to regularly evaluate the care delivered. 2.To liaise in a professional manner with other external professional agencies 3.To participate in the formulation and reviews of policies and procedures within the service. 4.Demonstrate the ability to evaluate information, make timely decisions and solve problems. 5.To maintain accurate records of appointments and clinical records 6.To collaboratively involve the person in their discharge planning and to include the involvement of other support agencies if necessary. 7.To demonstrate awareness and training within Childrens First guidelines and work in conjunction with these and subsequent reporting to relevant personnel. 8.To demonstrate the ability to resolve conflict successful and awareness of complaints procedure within the service. 9.To work within the quality standards set out by the Mental Health Commission and Best Practice Guidelines 10.To actively participate in the process of clinical governance and audit process. 11.To carry out specific assessments relevant to case load 12.Ability to work within a community setting and the capacity to provide and promote outreach services when necessary Education Component 1.To promote the role of Team Coordinator in CRS. Use personal initiative and a proactive approach in promoting the development of the service. 2.Demonstrate a commitment to professional development by attending relevant course days and keeping up to date with evidence based practice 3.To fully participate in information sharing with colleagues and presentation sharing. 4.To demonstrate a commitment to ongoing professional development and work on personal portfolio. 5.Ensure compliance with legal requirements, policies and procedures affecting service users, staff and other service matters. 6.Be familiar with the curriculum-training programme for student nurse and be aware of the clinical experience required to meet the needs of the programme. Foster a clinical learning environment. 7.To engage in performance review processes including personal development planning as appropriate. 8.To demonstrate an awareness of Freedom of Information and work within the remit of this. Audit & Research 1.Audit and research the service to evaluate the quality of patient care. Outcomes should contribute to the service plan, to the care of service users and families. 2.Ensure the views of service users and are effectively sought and integrated into the management and delivery of services.
