Location: Cluain Mhuire Services Initial Base: Rose Hill, Carysfort Avenue, Blackrock, Co. Dublin Cluain Mhuire is a national center of innovation in public mental health service provision delivering a community-based adult mental health service for the South East Coast of Co. Dublin to a catchment area of some 182,000 people approximately. In addition to general adult mental health services, Cluain Mhuire provides early intervention in psychosis, ADHD, gender, and general hospital mental health services. Clinical Nurse Manager 3 (Team Coordinator for the Crisis Resolution Service incorporating the acute Day Hospital) Full Time (37.5hrs) Permanent Post Saint John of God Community Services, Community Mental Health Services Cluain Mhuire Service are now inviting applications for the post of Team Co-ordinator (Clinical Nurse Manager 3) for the Crisis Resolution Service incorporating the acute Day Hospital. The successful candidate will be enthusiastic, flexible, innovative, and committed to providing a quality service that responds to the needs and priorities of our service users. Role Summary The CNM3 post in the Crisis assessment Service will act as a Team Co-ordinator for the Crisis resolution service. They will co-ordinate service development and provision across the Crisis assessment team and the Acute Day Hospital. They will assist the service in meeting the requirements of the national clinical programme for crisis services and provide professional/clinical leadership in their designated areas of responsibility. They will oversee the management of resources including staffing and staff development and facilitate communication across the healthcare teams. Minimum Requirements ELIGIBILITY CRITERIA Qualifications and/or experience: Registered in the Psychiatric division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnaimhseachais na hEireann) or entitled to be so registered. Have at least 5 years post registration experience of which 2 must be as a Clinical Nurse Manager or in a related area. Demonstrate evidence of continuous professional development at the appropriate level. Possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial, and administrative capacity to properly discharge the functions of the role. Health: Fully competent and capable of undertaking the duties attached to the office and in a state of health that indicates a reasonable prospect of ability to render regular and efficient service. Character: Must be of good character. Age: Age restrictions apply only to candidates classified as new entrants (within the meaning of the Public Service Superannuation Act, 2004). Principle Duties and Responsibilities Coordinating Clinical Team Ensure routine referrals are processed efficiently within the Crisis Resolution Service operational policy. Liaise with Primary Care Teams (GPs and primary care professionals), and other relevant local community services, external agencies, self-help, and other community resources. Co-ordinate agreed CRS clinical inputs and ensure all clinical/waiting list/admission and discharge documentation are completed by clinicians. Monitor monthly figures for the allocated MDT and ensure a plan is in place for delayed discharges/transfers. Assess, monitor, and escalate clinical risks relating to waiting lists/discharge lists/clinical issues and liaise with the consultant, Assistant Director of Nursing or nominated other regarding any risk issues and communicate risk management plans to the clinical team. Ensure all clinical incidents are reported and escalated appropriately and in a timely manner. Monitor team members' workloads in line with agreed discipline-specific workload guidelines. Co-ordinate team members' leave in accordance with the protocols agreed with relevant line managers. Work with members of the Multi-Disciplinary Team to implement the strategic objectives for the St John of God Community Mental Health Services. Maintain good staff relations within the team and promote good communication with all relevant staff and departments. Provide support and counsel to the multi-disciplinary team as necessary. Lead the development of local clinical operational procedures in line with the HSE CRS Model of Care. Supervision/Management Provide clinical supervision to student nurses/nursing as directed by the Assistant Director/Director of Nursing. Line manage nursing staff as directed by the Assistant Director/Director of Nursing. Contribute to the ongoing development of the nursing department. Escalate all issues related to Nursing to the Assistant Director/Director of Nursing. Clinical Practice Assess the needs and collaboratively prepare care plans for people attending the service. Liaise in a professional manner with other external professional agencies. Participate in the formulation and reviews of policies and procedures within the service. Evaluate information, make timely decisions and solve problems. Maintain accurate records of appointments and clinical records. Collaboratively involve the person in their discharge planning. Demonstrate awareness and training within Children's First guidelines. Resolve conflict successfully and be aware of complaints procedure within the service. Work within the quality standards set out by the Mental Health Commission and Best Practice Guidelines. Participate in the process of clinical governance and audit process. Carry out specific assessments relevant to case load. Ability to work within a community setting and promote outreach services when necessary. Education Component Promote the role of Team Coordinator in CRS. Demonstrate a commitment to professional development. Fully participate in information sharing with colleagues. Engage in performance review processes including personal development planning. Ensure compliance with legal requirements, policies, and procedures affecting service users, staff, and other service matters. Be familiar with the curriculum-training programme for student nurses. Demonstrate an awareness of Freedom of Information. Audit & Research Audit and research the service to evaluate the quality of patient care. Ensure the views of service users are effectively sought and integrated into the management and delivery of services. General Keep abreast of emergency procedures. Familiarize with the terms of the Safety, Health and Welfare at Work Act, and all relevant policy documents. Respect and operate within the framework of the tradition, character, and ethics which govern SJOG Community Mental Health. Maintain strict confidentiality regarding all matters concerning clients, staff, and business of the Centre. Undertake any other duties as may be assigned by line management. Remuneration Salary Grade 238Y CLINICAL NURSE MANAGER 3 (MENTAL HEALTH) on the Consolidated HSE Pay Scale €67,972 - €76,762 per annum pro rata. Initial location The initial location of this role is the Centre for Living, however, you may be required, from time to time, to work at SJOG CS's other places of business or/and the premises of such associated companies or organisations as SJOG CS may require.
