Applications are now invited for the following position: Clinical Nurse Manager I Primary Care Service (1.0wte Full-time, Specified Purpose contract) The main purpose of this post is to coordinate and deliver a Primary Care Service under the direction of the CNM2 in line with the physical needs of Service Users in SPMHS. To assist in the development and expansion of the service when required. To deliver defined care pathways for the service users. This role provides a great opportunity to learn and professionally develop your career pathway in SPMHS. It includes participating in all aspects of service delivery under the supervision and support of senior nursing staff. The role Clinical Nurse Manager I Primary Care 1.0WTE Full Time, Specified Purpose Contract The CNMI will coordinate and deliver a Primary Care Service under the direction of a General Practice CNM2 in line with the physical needs of Service Users in SPMHS. They will assist with the development and expansion of the service when required and deliver defined care pathways for the service users. The CNMI will report to the Director of Nursing (professionally) and General Practitioner in Primary Care (clinically). The standard working week applying to the post is 37.5 hours per week, Monday to Friday. Location The Primary Care Service (GP), St Patricks Mental Health Services, Jamess Street, Dublin 8, incorporating St Patricks Hospital, Lucan, and other services operated by St Patricks Mental Health Services. Responsibilities The duties of the CNMI are outlined below. In carrying out all duties, the CNM I must adhere to Hospital policy & procedures, health & safety requirements, and legislative requirements: Report to the CNM 2 Primary Care Nurse on a day-to-day basis and assist in the development and coordination of the Primary Care Service in consultation with the CNM 2, in line with international and hospital practice Process referrals from MDTs to the Clinic Assess/ triage all referrals with a method of prioritizing medical illnesses based on need Liaise with MDTs so that MDTs can develop appropriate care plans following assessment Arranging transfer of service users to an Emergency Department if required Take blood samples, administer wound care, and administer medications Maintain proper supplies of medical equipment Liaise with other hospital departments and GPs as appropriate Refer to Physical Health Programme as appropriate Liaise with ward managers and visit service users on wards when requested to assess medical needs Maintain patient, staff, and hospital confidentiality in accordance with hospital policy Comply with protocols, procedures, and safety policies of SPMHS Disseminate information and provide education to ward-based nurses, which is designed to improve knowledge and physical health care Identify needs and record outcome measures to ensure clinical excellence Know and adhere to the constraints of Nursing and Midwifery Board of Ireland (NMBI)code of professional conduct and scope of practise Liaise with other colleagues, and external agencies as appropriate Assume all other duties relevant to the role as may be required from time to time This job description is not intended to be a comprehensive list of all duties. Salary: The salary for the position is depending on experience and based on the CNMI 2024 salary scale (€54,832 - €64,616). Annual Leave The annual leave associated with the position is depending on post-registration experience. Pension: All pensionable staff become members of the pension scheme after successful completion of probation. The scheme is a defined contribution scheme with a deduction from the salary of the employee of 4%.In respect of an internal appointment being made, the individuals existing pension arrangements will continue. Probation: The appointee will be appointed in a probationary capacity for the first six months during which time the job holders performance will be subject to ongoing review. Any extension to or suspension of the period of probation will be in line with the European Union (Transparent and Predictable Working Conditions) 2022.
