TTM Healthcare in partnership with our client a group or nursing homes situated in Limerick, Kilkenny and Tipperary are actively recruiting a full time Clinical Nurse Manager for their 43 bedded nursing home based in Tipperary. Purpose of the Job The CNM role is to support nurses in planning and organising care and activities for residents at floor level. The CNM provides leadership, mentoring and supervision to nursing and care staff and monitors the safety and quality of care and services to residents at floor level. Key Responsibilities: Management and Leadership. 1. Act as a role model for registered nurses. 2. Monitor the competence of nurses and care assistants through informal observation and formal competency assessments as required. 3. Provide mentoring and coaching to nursing and care staff as the need arises throughout the shift. 4. Act as the team leader for the nursing team on a day-to-day basis. 5. Attending morning and afternoon handovers in order to prioritise and plan daily care activities with the team in accordance with each residents current care and condition. 6. Advise and support nursing staff with individual care plans on a daily basis to ensure that each residents care plan is updated in accordance with their changing needs. 7. Assist with performance management appraisals of nursing and care staff. Key Responsibilities: Professional and Clinical. 1. Promote a person centred approach to all aspects of assessment, planning and delivery of care to residents. 2. Coordinate the daily care activities for residents in accordance with each residents current condition and needs. 3. Promote and support nurses on evidence-based decision making for assessment and care planning. 4. Arranging and attend scheduled formal reviews of individual residents care plans. 5. Keep the director of nursing updated on individual residents care and condition, with particular reference to residents with unstable and / or unpredictable conditions; new residents and those in the terminal phase of end-of-life care. 6. Attend staff meetings as required by the person in charge and management team. 7. To liaise on a daily basis with the allied professional healthcare team including the general practitioners. 8. To perform pre-admission assessments in accordance with the admissions policy and procedures for potential residents as guided by the director of nursing. Key Responsibilities: Quality Management 1. Membership of the clinical governance committee. 2. Carry out audit activities in their area under the direction of the director of nursing, in accordance with the audit plan developed by the clinical governance committee. 3. Monitor the quality and safety of care and service delivery to residents through informal observation; audit findings; care planning activities and informal discussions with residents. 4. Reinforcement and monitoring of compliance with quality and safety systems, policies and procedures at floor level. 5. Provide feedback to staff on all risk management and quality activities and initiatives undertaken by the clinical governance committee. 6. Undertake audits of care plans under the direction of the director of nursing in accordance with the annual audit plan. 7. Assist the director of nursing in collating quality and safety data for the clinical governance committee. 8. Assist with the development and implementation of new and revised policies, procedures and guidelines for the nursing home. 9. Assist in the development and implementation of continuous professional development programmes for staff in the nursing home. 10. Maintain timely and accurate documentation related to health and safety; risk management and quality in accordance with national standards; regulations; local policies and professional guidance. 11. Report to the director of nursing any concerns regarding the quality of care and services to residents at floor level. Key Responsibilities: Risk Management 1. Implement risk management initiatives developed by the clinical governance committee. 2. Monitor and report any environmental hazards or risks in their designated area. 3. Supervise staff compliance with health and safety and risk management in their area. 4. Ensure incidents that occur during their shift are managed and recorded on Care Monitor appropriately. 5. Meet with the director of nursing following handovers to ensure that he/she is given an update on the status of residents and to agree on priorities for the shift. 6. Meet with nurses on each floor to plan care and activities for the shift, with particular emphasis on residents whose condition has deteriorated and who require additional care and monitoring during the shift. 7. Carry out a review on a weekly basis of all incidents that have occurred to ensure that follow up actions and all required documentation, including updating assessments and care plans have been completed. Key Responsibilities: Education and Training 1. Attend all mandatory in-service education programmes. 2. Participate in continuing professional development activities in the nursing home. 3. Provide induction guidance/training to new staff as identified in the staff induction programme. 4. Participate in performance appraisals as directed by the person in charge. 5. Report to the director of nursing any concerns regarding knowledge and competency deficits among staff, so as to inform education and training plans. Person Specification: Qualifications and Experience. 1. Be registered in the General Division of the Nursing and Midwifery Board of Ireland. 2. Preferably have a post registration qualification in management. 3. Preferably have a post registration qualification in nursing older people. 4. Evidence of Continuing Professional Development in the last two years. 5. At least 3 years post registration experience, of which 2 years include nursing older people. Person Specification: Knowledge, Skills and Competencies. Can demonstrate knowledge related to National standards and regulations for residential care services for older people. Professional codes and standards related to practice and to caring for older people. Auditing. Risk management. Health and safety. Can demonstrate: Management and leadership skills.
