Seeking a CNS and Staff Nurse to join a multidisciplinary team in Galway City, working on an Integrated Care Programme for the Prevention and Management of Chronic Disease.
Job Description:
- CNS (Cardiology): Work with a multidisciplinary team within an Integrated Care Programme designed to treat patients at the lowest level of complexity that is safe, timely, efficient and as close to home as possible.
- The Integrated Care Programme for the Prevention and Management of Chronic Disease (ICPCD) focuses on a number of chronic diseases that impact a large number of health service users.
Responsibilities:
- Work with a multidisciplinary team to provide care to patients with chronic diseases.
- Contribute to the development and implementation of the Integrated Care Programme.
- Collaborate with healthcare professionals to ensure safe, timely, efficient and effective care.
