Job Description

Overview of role Operating as a member of the Clinical Services Team, the Community Clinical Partner (CNM 1) will offer the support to and clinical oversight of services provided by Cheshire Ireland though supporting the frontline care support teams and Management in each service in the standardised implementation of organisational clinical policy and procedures, ensuring full compliance with organisational policy, statutory and legislative requirements and standards. Duties and responsibilities The identification of service users individual needs. The identification of the support required to meet these needs. Encouraging a culture that promotes better health and wellbeing for service users. Through clinical audit, evaluate services to assess how well they are meeting the identified needs of service users. Liaise with the Head of Clinical Services, Clinical Lead and the Service Manager where gaps are identified. Ensure consideration of National Clinical Guidelines and relevant policies and procedures when assessing and planning individual service users care. Provide support and advice to the services care teams in making decisions based on the best available evidence. Provide advice and support to service care teams so care is planned and delivered to meet the individual service users initial and ongoing assessed healthcare needs, while taking account of the needs of other service users. Education/Experience In possession of a nursing qualification and registered in the general division with the Nursing, Midwifery Board of Ireland (NMBI) (other divisions may be considered). Awareness of the National HIQA Standards for Residential Services for Children and Adults with Disabilities. Ideally has experience in an environment focussed on physical disabilities and neurological conditions in a management capacity. Have experience in the delivery of training to staff at all levels. Has strong communication skills both verbal and written and is able to effectively communicate with all grades of employees. Proven background in evidence-based decision making skills. Strong interpersonal and communication skills, both verbal and written. * Computer literate (knowledge of Microsoft Word and Outlook is essential) In possession of the necessary documentation to work and live in the Republic of Ireland and able to speak & write English fluently.
