Job Description:
Promote and support independent living and community integration for people with disabilities. Facilitate educational, developmental, recreational, and occupational programmes.
- Develop and deliver person centred programmes to meet service user needs.
- Key worker to facilitate Person Centred Planning process.
- Support individuals and groups in accessing community to meet identified needs.
Requirements:
- Minimum QQI Level 5 in Social Care or equivalent.
- Experience supporting adults with intellectual disability and mental health issues.
- Experience in Day Service Setting (1+ years).
- Programme facilitation skills and person centred planning.
