Job Summary: We are seeking a Programme Facilitator to promote and support independent living and community integration for individuals with disabilities in Ennis, Co Clare.
We are an independent voluntary organisation providing services and support for people with disabilities and their families for over 70 years. Our work positively impacts the lives of over 10,000 people with disabilities, equipping them with the confidence, skills, and self-belief to achieve their goals.
Job Description:
As a Programme Facilitator, you will be responsible for developing and delivering person-centred programmes to meet the needs of service users. You will work closely with individuals and groups to identify goals and supports required, and maintain records as necessary.
- Develop and deliver person-centred programmes to meet the needs of service users
- Be a key worker to service users to facilitate the Person Centred Planning process
- Identify goals and supports required by individual service users and maintain records
- Support individuals and groups in accessing their community to meet identified needs
- Observe all rules and regulations of the organisation
Requirements:
- Minimum QQI Level 5 in Social Care, or equivalent
- Experience supporting adults with intellectual disability and people enduring mental health issues
- Experience of working in a Day Service Setting (1 Year min)
- Experience of facilitating and implementing a person-centred plan
- Good report writing skills and IT skills
- Excellent interpersonal skills
- Full driving license
