Role Title:Registered Advanced Nurse Practitioner (RANP): Cardiology (Heart Support) Job Reference Number:000775 Closing Date:Sunday, 3rd November 2024 Purpose of the Role: The advanced practice service is provided by nurses who practice at a higher level of capability as independent, autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating, and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/memoranda of understanding. The RANP in Cardiology (Heart Support) demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership, and complex decision-making abilities. The RANP in Cardiology (Heart Support) practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI 2014), the Scope of Nursing and Midwifery Practice Framework (NMBI 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI 2017), and the Values for Nurses and Midwives in Ireland (Department of Health 2016). The RANP in Cardiology (Heart Support) service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence based health policy at local, regional, and national levels. The RANP in Cardiology (Heart Support) contributes to nursing research which shapes and advances nursing practice, education, and health care policy at local, national, and international levels. The role of the Cardiology ANP would include but not be limited to: Development of rapid access clinics to facilitate admission avoidance in cases of clinical deterioration where admission can be averted through early specialist input. Enable early discharge, by having adequate senior team members available to process the high-risk discharges in an early supported discharge clinic model thereby reducing the median length of stay and reducing readmission. Educate and train Candidate CNS and Candidate ANP staff members to facilitate the expansion of community-based services. Extend educationof staff in community-based settings e.g. step down facilities. Filter and apply clinical judgement to ensure appropriate deferral of lower risk patients to lower acuity community-based services and higher risk more complex patients to appropriate services either in the Hospital or in the community, as community services grow and are enabled to manage complex but clinically stable cases. The ANP in heart failure will provide autonomous, safe, efficient, evidence based advanced nurse led care to optimise the quality of life and health care experience of people with heart failure. This involves undertaking and documenting a complete episode of care (assess, diagnose, order investigations, interpret results, plan of care, treatment, discharge and follow up) according to collaboratively agreed protocols and scope of practice in the clinical setting demonstrating advanced clinical and theoretical knowledge, critical thinking and decision making skills. The ANP will guide and educate the team of CNS in the Heart Support unit, including those who have a multisite (hospital and community hub) role. They will enhance the education of staff nurses in acute medical wards, ED and in community-based settings on the early signs of decompensation, and the core nursing management of heart failure patients. This RANP role involves undertaking and documenting a complete episode of patient care, according to collaboratively agreed protocols and scope of practice in this clinical setting; demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision-making skills. The advanced practice role demonstrates a high degree of knowledge, skill and experience that is applied within the nurse-patient/client relationship to achieve optimal outcomes through critical analysis, problem solving and accurate decision-making (NMBI, 2017). Central to this is the provision of quality care, a safe environment and processes for patients by the use of evidence based clinical guidelines that address patient expectations, promote wellness and evaluate care given. The role will contribute to nursing research to shape and advance nursing practice, education and health care policy at local, national, and international levels Required: Be a Registered Nurse or Midwife on the NMBIs Active Register. Annual registration: Practitioners must maintain active annual registration on the Advanced Nurse Practitioner Division of the register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnimhseachais na hireann) for the role. Must possess the requisite knowledge and ability including a high standard of suitability and clinical, professional and administrative capacity to properly discharge the functions of the role. And Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC). Possess a Masters Degree (or higher) in Nursing or a Masters Degree which is relevant, or applicable, to the advanced field of practice. The Masters programme must be at Level 9 on the National Framework of Qualifications (Quality & Qualifications Ireland), or equivalent. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice, in addition to clinical practicum. Nurse Prescribing Registration. Nurse Prescribing of Ionising Radiation or agree to undertake within an agreed time frame, the nurse prescribing of Ionising Radiation Certificate. Advanced Health Assessment Certificate. Demonstrate I.T. skills including excellent keyboard skills and ability to manage data in spreadsheets, databases, and vendor programs. Experience in teaching and assessing junior staff/nursing students. Experience in teaching and assessing patients/families/professional caregivers. Demonstrated clinical expertise in Cardiology including expert knowledge of cardiac medications and recommended treatment pathways. Proficient in 12 lead ECG and arrhythmia interpretation. Desired: Working knowledge of healthcare patient record systems. Experience in developing clinical practice guidelines which are evidence based. Change management experience.
