**Job Summary:** Basic Grade Pharmacist required to provide clinical pharmacy services to patients in a hospital setting, with a focus on patient safety, medication management, and quality care. **Job Description:** The Basic Grade Pharmacist will be responsible for providing a ward-based clinical pharmacy service, predominately to the age-related rehabilitation and long-stay units. The successful candidate will assist in the provision of the dispensary, clinical, medication safety, and medicines information services as appropriate. **Key Responsibilities:** * Provide effective, efficient, and safe pharmaceutical care to all patients under their care in Peamount Healthcare. * Identify and address patient care and medication management needs where appropriate. * Advise and provide information to other healthcare professionals, including other pharmacists, to assist them in their care of the patient. * Participate in the development and expansion of ward pharmacy/clinical pharmacy services. * Provide a patient-focused clinical pharmacy service, including prescription review, patient counseling, and medication reconciliation on admission. * Act as a specialist resource for patients and other healthcare professionals. * Ensure the safe use of medicines by evaluating and managing risks associated with the use of medicines and participating in the hospital's Medicines Safety reporting process. * Develop and implement policies, procedures, and guidelines relating to medicines management. * Participate in multidisciplinary working groups and committees as required, both within Peamount Healthcare and with other organizations as appropriate. **Requirements:** * Be a registered pharmacist with the Pharmaceutical Society of Ireland (PSI) or be entitled to be so registered. * Possess the requisite knowledge and ability (including a high standard of suitability) for the proper discharge of the duties of the office. * Have an understanding of the HIQA Medication Safety Monitoring Programme in relation to rehabilitation healthcare settings. * Excellent interpersonal, communication, and presentation skills and an ability to communicate in a manner suited to individuals, target groups, and health professionals. * Ability to communicate with members of multidisciplinary teams and managers assertively and with diplomacy to ensure adherence with medication policies and clinical best practice. * Evidence of ability to empathize with and treat service users/residents, relatives, and colleagues with dignity and respect. * IT skills, including the ability to engage with information systems and data management tools, such as Cliniscript, Microsoft Word, Excel, and PowerPoint. * A flexible approach to carrying out the duties of the role and open to new ideas. * Demonstrates initiative, innovation, and problem-solving ability and ability to identify and implement solutions and change. * Demonstrates team working skills and ability to build strong working relationships with multidisciplinary team members and colleagues of all grades. * Evidence of leadership skills, demonstrating experience completing audits/projects to improve service provision. * The ability to effectively evaluate information and make appropriate decisions. * Evidence of effective planning, organizing, and time management skills. * Displays motivation and commitment to providing a quality pharmacy service. **Working Conditions:** * The hours allocated to this post are 35 hours per week with 7 hours as a standard working day. * The allocation of these hours will be at the discretion of the Department Head and in accordance with the needs of the service. * Under the terms of the circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster/on-call arrangements as advised by their line manager. * Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016). **Benefits:** * 27 days per annum pro rata based on contracted hours. * A period of 12 months probation will be served: On commencement of employment. * Fixed-term to permanent contract. * Permanent employees commencing in promotional posts will also undertake a probationary period relating to their new post. * Employees of Peamount Healthcare are required to be members of the Hospitals Superannuation Scheme. * Deductions at the appropriate rate will be made from your salary payment. * Maternity leave is granted in accordance with the terms of the Maternity Protection Acts 1994 and 2001. * Peamount Healthcare operates a Sickness Absence Management policy in line with the new Public Service Sick Leave Scheme as introduced in 31st March 2014. * Pre-employment health assessment prior to commencing
