Western Care is seeking a Social Care Assistant to support individuals with high and complex needs in County Mayo.
Job Description:
We are looking for a dedicated and compassionate individual to join our team as a Social Care Assistant. As a key member of our team, you will work directly with individuals, supporting them to gain valued social roles and participate in meaningful activities. You will also advocate on their behalf and support the development of their Individual Plans.
Key Responsibilities:
- Deliver person-centered care and support to individuals with high and complex needs.
- Develop and implement individualized plans to promote social inclusion and participation.
- Advocate on behalf of individuals to ensure their needs and preferences are met.
- Work collaboratively with colleagues and other disciplines to provide high-quality care and support.
- Contribute to a positive and supportive team environment.
Requirements:
- Experience in facilitating and supporting individuals with high and complex needs.
- A genuine understanding and interest in promoting social inclusion and participation.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
- Flexibility to respond to service demands.
- Working knowledge of HIQA standards and regulations.
- Good knowledge of IT skills.
Other Requirements:
- Full driver's license.
- Irish Garda Vetting and Reference checks.
- Overseas Police Clearance (if applicable).
