BROTHERS OF CHARITY SERVICES IRELAND WEST REGION TEMPORARY FULL-TIME TEAM LEADER (CNM1 or SOCIAL CARE LEADER GRADE) CREGORAN COMMUNITY SERVICES, GALWAY JOB REF: 129/2024W Location: Cregoran service is a developing community-based day and residential services in Claregalway and in the easterly periphery of Galway City. We provide individualised services, centre based and community based day and residential service to adults with high support needs, based on personal outcome measures. We strive to provide an excellent service, integrating the community as much as possible into the daily lives of the people supported. Candidates will be required to be flexible in this position, and will be required to work a variety of hours to meet service need, as appropriate. The successful candidate may also be required to work between the various community houses and day care programme areas associated with the Cregoran Service, depending on service need. The Role: The Team Leader is a pivotal role in managing and delivering quality services on a day to day basis. The successful candidate must provide leadership in the development of the service in line with Personal Outcomes Measures, HIQA standards, New Directions where appropriate, and all future service requirements The vision of the Cregoran Services is to support people with intellectual disabilities to achieve a full and valued life in their community. Our focus is to form a relationship / partnership with each individual and their family, to support people to be valued citizens in their local community and to have ordinary life experiences. We work to create opportunities for people who use our service to have valued social roles in their communities and to have the chance to form real friendships. It is the Team Leaders responsibility to ensure that the life of each individual reflects the Organisations core values by supporting each individuals development in the areas of: Dignity and respect Promoting Independence in the area of personal development, educational, employment, life skills and day to day living skills Developing friendships and relationships Making choices Community Networking Ensuring the physical, medical, social and emotional wellbeing outcomes of the people supported are observed. Reporting/Responsible To: Service Coordinator / Area Manager Qualifications/Experience/Skills: ? Candidates should have a relevant professional qualification to the Health or Social Care sectors i.e. an RNID/RMHN qualification or a BA in Applied Social Studies/Social Care. A management qualification is also highly desirable. ? Candidates must have a minimum of 3 years post qualification experience gained in services for people with intellectual disabilities. ? Experience of working with individuals who exhibit behaviours of concern and have complex needs is an essential requirement of the job. ? Experience in a management role is highly desirable. ? Candidates should be able to demonstrate leadership, communication and team working skills and be a flexible and dynamic individual who will relate to people in a range of situations with an understanding of and commitment to the principles of public accountability as they apply in the health service. ? Candidates must have a full clean driving licence. ? Fluency in verbal and written English is an essential requirement of this post. Working Hours: The Brothers of Charity model of service is based on Personal Outcome Measures requiring employees to be flexible in their working hours to provide a quality service delivery for each individual. 75 or 78 hours per fortnight based on the successful candidates qualification status, over a 14 day duty roster. Contracted hours of work are liable to change over 7 days to meet the requirements of the service, to include days, evenings, weekends, public holidays, and night duty as service need requires. The successful candidate will also be required to take responsibility as the senior person on duty in order to support colleagues across Cregoran Services, on a rostered basis, as required. Annual Leave: Leave will be granted according to the agreed leave assigned to the grade of the successful Candidate i.e. CNM1 or Social Care Leader. Remuneration: Based on the salary scale attached to the grade of the successful candidate i.e. CNM1 or Social Care Leader. Probation: A probationary period of nine months from the date of appointment applies to the post. The employment may be terminated at any time during the probationary period should the employer find that the appointee is unsuitable to continue employment. The probation period may be extended at the Employers discretion. Tenure: This post is Temporary full-time and pensionable linked to a permanent staff members temporary move to another post within the services. JOB DESCRIPTION The Brothers of Charity model of service is based on Personal Outcome Measures requiring employees to be flexible both in their approach, and in their working hours to provide a quality service delivery for each individual. Specific Conditions Staff must have a positive attitude towards working with persons with an intellectual disability and help to ensure that they lead as normal and fulfilling a life as possible. Staff must treat each person with an intellectual disability as an individual, and at all times acknowledge and respect the rights and personal dignity of the person. Staff are required to establish and maintain relationships with adults with disabilities that are based on respect and equality and that promote their independence. The Team leader will work as a frontline member of the team with protected time to carry out specific team leader duties. Main duties, functions and responsibilities in cooperation with Area Management: Team Member Responsibilities and Relationships: ? Be an active member of the assigned service Team, while also being an active member of the Cregoran Services management team. ? Co-operate with all staff in maintaining, developing and enhancing the partnership model. ? Develop and foster good relationships with people supported in the service. ? Support and Supervision: Manage and supervise the performance of staff assigned to the service area including responsibility for their performance and conduct as well as timekeeping, attendance and planning annual leave. ? Co-operate on all aspects of programmes with the staff and the multidisciplinary team members. ? Assist in formulating, implementing and evaluating service plans in co-operation with the Service coordinator, multidisciplinary supports and other team members under the direction of the Service coordinator. ? Manage and promote community liaison including advocacy, citizen and advocacy. ? Promote and maintain positive family contacts with the families of all people supported. ? Ensure appropriate individual documentation is maintained to facilitate the completion of multidisciplinary reports. ? Coordinate the filing and storage of individual records in line with Organisational policy. ? Ensure that all IP records, local reviews, Team based performance records are completed and inputted on the database as required. ? Formulate and implement programmes designed to meet the needs of the individual, in cooperation with the multidisciplinary team as required, to ensure that each individual receives a quality day programme. ? Promote community integration and maximum participation for the individual. ? Ensure that people supported are empowered to control their own monies as far as is possible, and that appropriate individual financial records are maintained in line with Organisational policy. ? Ensure that all resources are managed effectively in order to achieve maximum efficiency in resource usage. ? Facilitate co-ordination, co-operation, liaison and communication across multidisciplinary and programme teams. ? Plan and record the discussion items of Team Meetings. ? Assist individuals in the planning of menus, preparation and cooking of meals, while ensuring food hygiene standards are maintained as per Organisational policy. Leadership and Management Responsibilities: ? Ensure all relevant individual files and required individual information is regularly reviewed, and is clearly and effectively communicated across the local team. ? Ensure the OLIS Database is regularly reviewed and updated as required. ? Take responsibility for the duty roster and ensuring staff cover in the case of absenteeism and ensuring return to work discussions are undertaken in a timeous fashion. ? Assist in managing all resources efficiently and effectively within agreed budget. ? Assist people who use the service with personal banking and adhering to the National Guidelines on managing individual finances. ? Provide leadership and direction for all members of the team to ensure good practice according to Organisational policy. ? Ensure that all aspects of service delivery conform to meet stated New Directions themes and/or HIQA regulations and standards as appropriate. ? As required, meet with and report to HIQA Inspection teams ? Where required ensure appropriate information is gathered and inputted to the services New Directions Easi-Tool, with regular review during intervening periods. ? Provide induction for new staff as required. ? Plan and provide support on the day to day operational and development of the service; within the Team Based Performance Management framework. ? Provide leadership, good time management and supportive supervision to staff in the service where appropriate, and appraise staff performance as required. ? Provide support to manage and resolve problems within the team when appropriate. ? Facilitate team building and staff motivation. ? Ensure all interactions are grounded in positivity, dignity and respect for all stakeholders at all times. ? Promote an atmosphere of inclusiveness, continuing development, and mutual support, grounded in respectfulness at all times. ? Participate in the identification, development and delivery of education, training and development programmes for staff. ? Ensure the appropriate proactive strategies are documented and implemented for each person supported by the service. ? Assist in supervising and assessing volunteers and students as required. ? Lead and implement change. ? Be proactive and take an active part in future service planning, development and delivery. ? Perform such other duties appropriate to the post as may be assigned from time to time by the Service Co-ordinator/Area Manager. ? All duties must be carried out in person at all times. ? The duties and responsibilities of all posts within services are likely to change to meet the developing or changing needs of the people being supported, staff are required to have a high level of flexibility, and a willingness and ability to develop new approaches to their work as required. Health, Safety and Hygiene Responsibilities: ? Participate in assisting people supported with personal hygiene and personal care and promote their independence. ? Implement all public health advice as regards the prevention and management of notifiable diseases. ? Provide leadership and direction for all members of the team, to ensure that high standards of cleanliness are maintained according to the services hygiene and infection control policy. ? Record and audit hygiene practices in the assigned service area. ? Ensure that effective safety procedures are developed and managed to comply with the statutory obligations in conjunction with relevant staff. ? Ensure all Fire Drill procedures within the Centre,are undertaken as per Organisational Policy i.e. fire detection, evacuation and fire fighting. ? Ensure all individual Personal Emergency Egress Plans (PEEPs) are reviewed and in place. ? Ensure all regulations are adhered to with regard to transport and insurance ? Be conscious of Health & Safety matters in the workplace and, in particular, to comply with employees' obligations as set out under Chapter 2 of the Safety, Health & Welfare at Work Act, 2005. ? Ensure that all accidents/incidents to the people supported, staff or visitors are reported in accordance with Service policy and that all staff are fully aware of the contents of the Safety Statement for the Centre. ? Carry out regular review of AIRS in line with the multidisciplinary team reviewing and planning protocols to meet any highlighted trends and patterns. Further responsibilities: ? To monitor any medical needs of people supported attending the centre. ? To be responsible for the administration and ordering of medication in line with the medication administration policy of the service. ? To perform or arrange appropriate procedures as required and recommended by the medical department of the service, or other medical personnel involved with the individual. ? To ensure that medications and oxygen, where required, are safely and appropriately stored. ? To ensure that any medical equipment is clean and in working order. ? Supervise and support colleagues who have completed the two day Safe Administration of medication. (S.A.M.S.) and support them with carrying out their practical assessments. ? Ensure that specific medical and nursing supported care plans are formulated, implemented, reviewed and updated as required. ? Ensure regular consultation and follow up with GPS, specialist practices and mental health services. ? Ensure everyday care practices are upheld are in line with health promotion within a community setting ? Ensure further educational and skills based training is undertaken to improve clinical practice. ? Undertake educational on line training modules as required. ? Ensure individualised care plans are relayed to support staff. Personal Outcomes Responsibilities: ? Manage the supports provided to people who use the service, ensuring care and learning experiences are of the highest professional standards, using evidence based planning approach such as an individual personal outcome for each person supported. ? Actively support Key Workers in their endeavours to achieve priority goals for people supported within the service. ? Assist people supported in the service in making informed decisions. ? Facilitate individual plan meetings and review meetings where required. General Responsibilities: ? Ensure strict confidentiality in all matters concerning people who use the service including the safe storage of reports and records. Ensure all information is adapted as needed. ? Driving the Service vehicle as required. ? Ensuring that all service equipment (including vehicles) are maintained in a good state of repair, arranging servicing and maintenance as required. ? Evaluate and manage the implementation of best practice policy and procedures including the policies and procedures of the Brothers of Charity Services and operate within the policies and procedures of the Services in all aspects of your work. ? Promote dignity at work and show respect to people who use the service, colleagues and stakeholders in the course of duty. ? Avail of all training and development opportunities that enhance this position. The role of the post holder will not be limited by reference to this job description. It is expected that the role will evolve and develop as required by professional, structural, and the people supported by the services demands, requiring the post holder to demonstrate a high degree of flexibility to facilitate this.
